Discovery Questionnaire

Before we meet — I want to see you.

I spent years performing a version of myself that fit what others expected — and it cost me the very thing I was working so hard to protect: my authentic voice.

What I know now is that the work of growing into yourself begins with the courage to tell the truth about where you are. That's what this intake is asking of you.

There are no right answers here. There is only your truth — and the more honestly you give it to me, the more powerfully I can show up as your Accountability Advocate on our call and throughout our work together.

Take your time. Sit with the questions that make you pause. Those are usually the most important ones.

“The most powerful thing you can grow into is yourself. This is where that begins.”

01
Open-ended

Before we talk about where you want to go, I want to understand where you are.

In your own words — not what looks good on paper — how would you describe where you are right now, professionally and personally?

What made you say yes to this intake today — and what almost made you say no?

02
Scaled

On a scale of 1–5, how authentically do you feel you show up at work right now?

I've worked with women who were brilliant, capable, and completely invisible in the rooms that mattered most. Not because they lacked talent — but because they had learned to shrink.

1 = I perform a version of myself     5 = I show up fully and unapologetically

What does that number feel like in your daily experience?

Give me a specific moment or situation where it shows up most.

03
Open-ended

Growth that doesn't stick isn't growth — it's a temporary lift.

I want to know what's already been tried. What have you already done to try to move forward — a course, a book, a mentor, therapy, another coach? What happened, and why do you think it didn't stick?

04
Open-ended

Something brought you here right now — not six months ago, not next year.

What is the moment, conversation, experience, or feeling that made right now the time to invest in this work?

What happens if nothing changes in the next six months?

05
Scaled

On a scale of 1–5, how do you typically respond when someone challenges your thinking or offers feedback you didn't ask for?

One of the most important things I do as your Accountability Advocate is push back — with care, but without softening what needs to be said.

1 = I shut down or withdraw     5 = I lean in and use it

Describe how that actually shows up for you.

What does receiving hard feedback look or feel like in your body and in your behavior?

06
Reflective

We all learn differently — and the best coaching meets you where you are.

How do you learn best? Do you process by thinking quietly and coming back? By talking it out in real time? By reading and reflecting? By doing and then adjusting?

Think about a time you successfully changed a behavior or habit.

What made it work for you?

07
Open-ended

What genuinely motivates you?

I believe that what lights us up and what shuts us down are equally important maps to who we are. What genuinely motivates you — not what should motivate you, but what actually does? And on the other side, what discourages you most or makes you want to disengage?

08
Reflective

What does fear look like for you?

Fear has a way of disguising itself — as procrastination, perfectionism, over-preparation, or silence. I've worn all of those masks myself. How does fear show up in the moments when something important is at stake?

When things get hard — what is your pattern?

At work, in a relationship, in a commitment you've made to yourself — do you push through, pull back, get busy, go quiet?

09
Scaled / Reflective

Six weeks from now, what would make you say “this worked”?

Success and failure mean different things to different women — and how we define them shapes everything about how we pursue growth. Describe what success looks and feels like for you at the end of this program — in your career, in how you show up, and in how you feel about yourself.

Now the harder question — what would failure look like?

What are you most afraid won't change?

10
Reflective

What kind of support do you need most right now?

The last thing I want to know before we meet is what you need from me — because advocacy without understanding is just pressure.

Is there anything you want me to know about you?

About how you're wired, what you've been through, or how to show up for you — that this form didn't ask?

Before you submit

Thank you for trusting me with your truth.

I read every word of every intake personally. Not as a form — as a first conversation. By the time we get on our discovery call, I will already be thinking about you, your patterns, and how I can best show up as your Accountability Advocate.

I'll be in touch within 48 hours to confirm your discovery call time. Come ready to go deeper.

Your truth is where the work begins.
